Goshen Medical Center, Inc. is a private, non-profit 501(c)(3) organization and Federally Qualified Health Center (FQHC) that has served eastern North Carolina residents since 1979.

4. Patient Portal
We provide a secure patient portal for appointment scheduling, messaging, and access to portions of your health information.
By using the portal, you agree that:
- It is not monitored continuously and must not be used for emergencies
- Messages may be reviewed by clinical and administrative staff for care coordination
- Communications become part of your designated medical record
- You are responsible for maintaining the confidentiality of login credentials
- Access may be suspended or terminated for misuse or security concerns

5. SMS Notification Service for Board Members
5.1 Service Description
Goshen Medical offers an SMS notification service exclusively for current Board Members to receive meeting reminders and governance-related updates.
5.2 Eligibility and Consent
- Participation is limited to active Board Members
- Explicit opt-in consent is required via Microsoft Forms
- Consent is not a condition of Board service
- Members may opt-out at any time
5.3 Message Frequency and Charges
- Message frequency varies based on Board schedules
- Goshen Medical does not charge for SMS notifications
- Standard carrier message and data rates may apply

Participation in surveys is voluntary. By submitting feedback, you grant permission for Goshen Medical to use responses for quality improvement purposes.

Goshen Medical complies with the Health Insurance Portability and Accountability Act (HIPAA) and applicable North Carolina privacy laws.
